Imua Physical Therapy’s health insurance plan provides employees access to medical, dental, and vision care insurance benefits. Employees in the following employment classifications are eligible to participate in the health insurance plan:

* Regular full-time employees

* Regular part-time employees (as described in this manual)

Eligible employees may participate in the health insurance plan subject to all terms and conditions of the agreement between Imua Physical Therapy and the insurance carrier. The employee’s contribution will be 1.5% of the employee’s monthly wages.

UHA (University Health Alliance) is Imua Physical Therapy’s Medical Insurance Provider. The plan that we subscribe to is UHA 3000. UHA is also Imua Physical Therapy’s Vision Care provider. The plan that we subscribe to is UHA Vision Plan 100. The Dental Plan is called Dental Plus through UHA.
