When you call, we will collect basic information and begin the registration and insurance verification process prior to your first scheduled visit. Our primary forms are available for download, or if you choose, we will mail, fax or email the forms to you – whichever is most convenient.

At Imua, you have the option of being treated without having a physician’s referral or prescription, although many insurance plans will not pay for rehabilitation services without one. To obtain a written referral, please consult with your physician or medical provider.
